HIV Case Manager

"The Case Manager functions as the leader of a three-person intensive case management team, which coordinates comprehensive services for a caseload individuals and families living with HIV/AIDS. This involves coordination of health care and social service linkages, assistance with entitlements, housing and ongoing psychosocial support. Primary Responsibilities: Assess clients' needs for services and provide intervention accordingly. Develop and maintain treatment plans with clients. Maintain documentation including case conferences, progress notes and correspondence. Establish linkages with other community based organizations and work toward improving service coordination, access, and delivery of services. Conduct home and hospital visits and monitor medical conditions of clients."
